When a cavity gets close to the nerve of a tooth, a root canal isn't always the only option. In many cases — especially in children and in permanent teeth with immature roots — a more conservative treatment called vital pulp therapy can save the tooth's nerve entirely, avoiding a root canal altogether.

What Is Vital Pulp Therapy?

Vital pulp therapy is a treatment aimed at preserving the health of the tooth's pulp — the soft tissue inside the tooth containing nerves and blood vessels — when it's been affected by deep decay or injury but is still alive and healthy enough to heal. Rather than removing all of the pulp tissue, as a root canal does, vital pulp therapy removes only the portion that's been compromised and protects the remaining healthy tissue so it can continue to function normally.

What Is a Pulpotomy?

A pulpotomy is one of the most common forms of vital pulp therapy. During the procedure, Dr. Shah removes the affected pulp tissue from the crown (top portion) of the tooth while leaving the healthy pulp in the root intact. A medicated material is then placed over the remaining pulp to protect it and encourage healing, and the tooth is sealed with a filling or crown to prevent further decay or contamination.

When Is This Treatment Used?

Vital pulp therapy and pulpotomies are most commonly recommended for:

  • Primary (baby) teeth with deep decay, allowing the tooth to stay in place until it naturally falls out
  • Permanent teeth in children and teens whose roots haven't fully developed yet, since preserving the pulp allows the root to keep maturing
  • Teeth with reversible pulp inflammation, where the nerve is irritated but not yet permanently damaged

How It's Different From a Root Canal

A root canal becomes necessary when the pulp is too damaged, infected, or dead to be saved, requiring removal of all the pulp tissue from both the crown and the root canals themselves. Vital pulp therapy, by contrast, is only possible when enough healthy pulp tissue remains to heal on its own. Because it preserves living tissue rather than removing it entirely, vital pulp therapy is a more conservative treatment — but it's only an option when the tooth's nerve is caught in good enough condition to save.

Why Preserving the Pulp Matters

Keeping pulp tissue alive and functioning has real benefits, particularly in children. A tooth with intact pulp can continue developing normally, including further root growth in young permanent teeth. This is especially important for the long-term stability and strength of a child's adult teeth as they continue to mature.

What to Expect During the Procedure

Vital pulp therapy is typically a comfortable, single-visit procedure performed under local anesthesia. After the affected pulp is removed and a protective medicament is placed, the tooth is restored with a filling or, in some cases, a crown for added protection. Most patients experience little to no discomfort afterward, and the tooth is monitored at future visits to confirm it continues to heal well.
